Our Facility
At Liberty Wellness, your privacy is important to us. From the first phone call to completion of the program, your treatment experience is 100% HIPPA protected and confidential. We can however provide progress updates, completion letters, and other information upon your request and signature on a Release of Information form for a person/organization of your choosing. Without your consent through this release form, we will not provide anyone with information about your treatment with us.
All potential clients will complete a pre-admission assessment, usually by phone, which is reviewed by the Clinical Director and/or the Medical Director to measure eligibility to the program. The pre-admission assessment includes basic demographic information, medical history, substance use and mental health history. The call is 100% confidential.
The Intensive Outpatient Program (IOP) typically lasts between 8-12 weeks depending on individual needs, prior to stepping down to the Outpatient Program (OP). The OP program lasts at least 8 weeks as well, but may be longer depending on the continued progress of the individual. Upon completion of OP, individuals are strongly recommended to follow their aftercare plan, which may include engagements such as recovery support groups, meeting with a psychiatrist, and participating in wellness activities.
Program Description:
Upon admission into the program, clients will be placed with a primary therapist who will work with the client throughout their treatment. Group sessions will consist of recovery, mental health, life skills, wellness, and process groups. Individual sessions are where the client will identify personalized treatment goals to work on throughout treatment. Clients will be encouraged to include family members to participate in at least one family session during their episode of care. Additional family sessions are available based on clinical necessity. Each client will receive a psychiatric evaluation, and will receive medication monitoring as needed by our medical staff throughout treatment.
Program Layout
This is a co-occurring program that focuses on substance use disorders as well as other mental health disorders. There are three types of groups that we touch on every day. The three types of group are substance use/mental health, wellness/life skills, and process/support.
Substance Use/Mental Health Groups: These groups are aimed at treating substance use disorders and co-occurring mental health disorders. The focus of each of these groups will be modified continually based on the needs of those in the program. Once per week, one of these groups will be a trauma group using the curriculum from Seeking Safety. Below are some examples of substance use/mental health groups that will be offered:
Substance Use: Psych of Addiction, MATs, Triggers, Coping Skills, Support Systems, Relapse Prevention
Mental Health: Trauma, Grief and Loss, Anxiety, Anger Management, Medications, Depression
Wellness Groups/Life Skills Groups: These groups are aimed at helping individuals develop five core areas of wellness. Every week will focus on an area of wellness and this will occur in repeating five week cycles. The focus of each wellness group will be modified continually based on the needs of those in the program. Below are some examples of wellness/life skills groups that will be offered:
Psychological/Spiritual Wellness: Values, Gratitude, Purpose, Forgiveness, Self-worth, Acceptance Relational/Social Wellness: Connection, Conflict Resolution, Sexuality, Empathy, Parenting, Listening Physical/Biological Wellness: Nutrition, Preparing Meals, Sleep Hygiene, Exercise, Medical Basics Vocational/Financial Wellness: Career Development, Resumes, Budgeting, Educational Development Leisure/Recreational Wellness: Relaxation Skills, Time Management, Sober Fun, Boundaries with Electronics
Process/Support Groups: These groups are more flexible groups where clients engage in exercises such as a check-in, relapse prevention for the weekend, peer support, gender groups, and creating/sharing personal life timelines. The following is an example of what a weekly schedule could look like.
